Indicate approximate date(s) of the incident.


Describe in detail the facts or events that occurred to help analyse and evaluate the complaint.
Your information is valuable and will facilitate our investigation, so it is important that you can mention it in your report:

· The detailed description of the event, the place where it occurred.
· The date(s) and if possible the time.
· Names and positions of who is responsible and/or involved.
· Who else knows about it.
· Where we can gather further information or evidence of what is reported.
· If you have physical or digital evidence, please provide it.
